By submitting a booking request, you release me of any and all liability for your mental or physical health, safety, injury, infection, or accidents that may arise before, during or after our time together. That being said, I am experienced and attentive, and in all my years of lifestyle and pro domination, there has never been a serious injury. If you have an extreme request, I will require a specific liability waiver, which I will commission at your expense.

Complete the supplicant vetting form and text the number provided to find out if you are approved, so you may select your session time. Ask permission before using a provider as a reference and include their website, twitter or contact info. Being on a review board is not an acceptable form of screening. If you are accepted, you will be informed of any additional screening information I may require.
